In order to do COVID-19 exposure contact tracing and collect information on all prospective families, it is critical that you submit the sign-in sheet information you collected electronically below AND mail the sign-in sheet to the St. Paul Service Center.
If you collected any short forms, please submit them electronically below.
Your Sign-In Sheet, a Member Form Cover Sheet, and any full paper membership forms and payment should be mailed using your pre-paid envelope to the St. Paul Service Center (400 Robert Street South, St. Paul, MN 55107) within 48 hours of your event.
